We put together this plan to provide a cost-saving alternative for cash, patients’ credit dental card, needs. These options are paid one time annually and can be paid by debit card, or check. - 2 cleanings per year child/adult
- 2 periodic exams per plan year
- 2 problem focused exams per plan year
- 4 bitewing X-ray films per plan year
- 4 periapical x-ray films per plan year
- 1 full mouth x-ray (usually taken every 3-5 years)
- 2 flouride treatments per plan year

- Invisalign Express – $200 Off
- Fillings – 15% Off

This plan CANNOT BE USED:
- In conjunction with another dental plan
- For services for injuries covered under Workers Compensation
- For treatment which, in the sole opinion of the treating dentist or doctor, lies outside the realm of his or her capability
- For referrals to specialists outside our office
- For hospitalization or hospital charges of any kind
- For costs of dental care which are covered under automobile medical
- This is NOT dental insurance and will only be honored at our office.
- The plan cannot be used in conjunction with any insurance coverage.
- Premiums must be paid in full with cash, check, or credit card only.
- You are free to cancel the plan at any time, but there are NO REFUNDS of any kind.
- You are expected to pay your full portion for treatment AT THE TIME OF SERVICE.
